Eligibility

The American Diabetes Association Summer Camp Best ID conest by American Medical ID is open to all American Diabetes Association Summer Camp participants at  previously selected ADA camp sites.  All employees of American Medical ID and their family members are excluded from this contest.

Contest entrants under the age of 13 must have permission from a parent or legal guardian to enter.

How to Enter

After creating your American Diabetes Association Summer Camp Build-an-ID, take a picture and submit it to American Medical ID through www.IdentifyYourself.com/ADACamp.

All pictures and statements submitted become property of American Medical ID and can be used in future promotional material.

Only one entry allowed per person.

Grand Prize

One Grand Prize winner will receive a $1,000 savings bond to be used for a scholarship.  The Grand Prize winner and other top entries will have their ID and/or statement used for future promotional materials.

In order to receive the Grand Prize, entrant must have a valid United States Social Security number.

Prize winner is responsible for all incidental expenses incurred in connection with accepting the prize.

Deadline

All entries must be received by September 30, 2007.  Winner will be announced and contacted by October 8, 2007.
