Participants can maintain an online personal health record of their medical conditions, emergency contact information, prescriptions, over-the-counter medications and personal and medical files such as living wills, donor cards, EKGs and CTs. This file can be accessed anytime through a secure online connection or a 24-hour emergency telephone line.

If you would like to have your personal health record access information engraved on your medical ID, you must enroll in the Online Medical Registry and obtain your access code prior to ordering your medical ID. Once you complete enrollment and obtain your unique access code, you may then order your medical ID to include the access information in the engraving section of the order process.

The other option is to order an Online Medical Registry charm to easily attach to your medical ID. This allows you to include more vital medical information on your actual ID and still include the Online Medical Registry access information. This also allows you to order your medical ID prior to enrolling in the Online Medical Registry.
