You may print the order form, then complete the product selection box, engraving information, shipping instructions and payment information. Payment can be made with a major credit card. Fax your order directly to us at 713.695.7358.

Order by Mail

Print the order form. Fill out the form completely, then mail it to us along with engraving, shipping and payment information. Payment can be made with a personal check, money order or major credit card. Mail your completed form to:

Order Online

Our online store features hundreds of medical ID bracelet and necklace styles to chose from! Once making your ID selection, you will need to provide instructions for custom engraving the ID. Online purchases can be paid for with any major credit card.

Order by Telephone

Call our toll free number, 1.800.363.5985, and place your order directly with a representative. Phone orders require payment with a major credit card. COD orders are not accepted.

Most orders will be shipped within two weeks. Rush processing and shipping available with additional cost.

Please note that all offers to purchase goods are only valid upon acceptance of same by the company at its head offices in Houston , TX . Upon acceptance of same, a legally binding sale of goods agreement shall exist. Sales of goods by the company to customers shall be on an ex warehouse basis.
